I agree to create and submit original raw (non-edited) video footage as outlined in the UGC Participant Guidelines. This includes footage of product preparation, cooking process, taste testimony, and finished plated product. All raw footage must be submitted to our marketing team via the private Google Drive link provided by Forlini’s Kitchen, by the date specified in the email. No AI generated content is permitted or accepted.

2. Approval Process

All submitted content is subject to review and editing by Forlini’s Kitchen marketing team prior to any public sharing. Forlini’s Kitchen marketing team will edit the final video, which may include the addition of closed captions and my social media handle. I agree not to post, share, or publish any raw or edited footage related to this program until I have received the final approved version from Forlini’s Kitchen marketing team.

3. Sharing Requirement

Upon receiving the final approved video, I agree to share it on my personal Facebook page and tag Forlini’s Kitchen. I agree to share only the final approved version and will not post raw or unapproved content related to this program on any platform so that I will align to their brand guidelines.

4. Grant of Rights

I hereby grant Forlini’s Kitchen, its owners, agents, and representatives a non-exclusive, royalty-free, worldwide, perpetual license to use, reproduce, edit, distribute, and display any content I submit, including video, photo, audio, name, likeness, and social media handle, across all platforms and media including but not limited to Facebook, Instagram, TikTok, and the Forlini’s Kitchen website.

5. Right to Decline

Forlini’s Kitchen reserves the right to decline to use any submitted content at its sole discretion, without obligation to notify the Participant. Forlini’s Kitchen also reserves the right to choose which portions of submitted footage are included in the final edited video.

6. Complimentary Meal

I understand that my participation is voluntary and that I will receive one single-serving complimentary menu item as outlined above. No monetary compensation, future discounts, or additional credits are implied or guaranteed. I will pick up meal at Forlini’s Kitchen location on date determined.

7. Content Ownership & Originality

I confirm that all submitted content is original and created by me. I have not included any third-party copyrighted material without permission. I am solely responsible for any content I produce and submit. I did not use any AI generated content and only submitted raw video as stated in 1. Content Submission

8. Food Safety & Liability Disclaimer

Forlini’s Kitchen products are prepared in a licensed commissary kitchen using fresh ingredients and are provided to Participant in proper condition at the time of delivery or pickup. Forlini’s Kitchen makes no representations regarding the safety of the product once it leaves our custody.

Participant assumes full responsibility for the proper storage, handling, and preparation of all food products received. Forlini’s Kitchen is not responsible for any illness, adverse reaction, or injury arising from improper storage, handling, undercooking, cross-contamination, or any other preparation method used by the Participant in their home.

By signing this form, Participant acknowledges that any foodborne illness or adverse reaction may result from factors outside of Forlini’s Kitchen’s control, including but not limited to personal food allergies, other foods consumed, improper refrigeration, or user handling error.
